What exactly is fascia and why do western medicine and TCM deal a lot with it in pain management?Its a membrane over the soft tissues such as the muscles and tendons which allows muscles to function as a group, for example, the flexor/extensor muscle groups. Fascia and soft tissue systems has been getting a lot of attention from both TCM and other medical practitioners because it is responsible for the majority of chronic pain issues.

Does the pelvic have to be completely aligned to be normal?There is a wide spread notion that for the human body to be healthy, all of the vertebrae and especially the pelvic needs to be perfectly aligned, meaning there is no tilt or rotation to the left or right. The key to why the pelvic as well as the vertebrae will have rotation is that none of us use both arms (and legs) with equal frequency and strength. For right hand dominant people, the pelvic will rotate towards the right and vice versa. This difference in usage is the reason why although the pelvic can be adjusted to temporarily align with both sides at same height, once you start to use it the muscle imbalance in the body will pull the pelvic back to its original position. Which is why you will find that although you keep getting adjusted, the alignment will always be off the next time you consult. So if rotation of the pelvic is within acceptable range, there is no need to keep forcing it back to position, and even when there is a need for the pelvic to re-align, it makes more sense to restore overall muscle balance so the muscle doesn’t pull the structure back after treatment.

What is TCM Soft Joint Adjustment?Its something that gets asked a lot, basically joint adjustments are techniques that uses the hand to change the position of bone, fascia, muscle and even the skin in order to achieve a certain structure. Soft meaning the technique is gentle, as opposed to hard techniques which utilize a sudden burst of strength. The advantages of soft adjustments are that its safe, as well as effective in achieving long term relief as all of the structures (including skin, muscle) can be aligned/released together in one direction.

What is muscle release?A muscle fibre is a 3D structure and if it gets too tight, it will be twisted and in a "knot". Very often we are told that if we massage or stretch it often then it will become soft and relaxed. However, many will realize that muscles that are massaged often do not necessarily become relaxed, this is because the "knot" needs to be released in a specific direction and depth. A muscle that is released will be in a relaxed state and hence the pain will be reduced significantly.

"My friend told me she was healed in one session"Its a common expression for many new patients and it often gives an unrealistic timeline for recovery. There are certainly cases where the patients feels much better after one session, but these are often cases involving a single joint, for example, a recent sprain to the ankle. However, the majority of cases coming in are chronic pain that have lasted for months, if not years. The arch of the foot, the pelvic ,ribs and fingers have hardened and mobility is low, many patients have already undergone numerous treatments and even surgery. Soft joint adjustment is a very meticulous craft that gradually softens and reduces tension from the skin to the underlying deep fascia and it requires a lot of effort on the part of the physician to achieve mobility in regions where the fascia has been hardened for years. For many patients with chronic pain, the effect is only more pronounced from the 3/4th session onwards. There are many common methods that produce faster results to soft joint adjustment, however, the relief may not last for long as the overall fascia structure has not changed. It is up to each individual which therapy they would like to undergo, but for each one that they choose, it is imperative that patients do have an understanding for the timeframe needed. A lot of patients may have gone to other therapies for months, but expect immediate results when they come. For simple cases, soft adjustment can yield immediate relief for some, but for complex cases, more time is needed.
